WellDENT Patient Charter
 

WellDENT established clear policy to protect your right:

  • You have the right to be given a clear description of your medical condition, with diagnosis, prognosis, the treatment proposed including common risks and appropriate alternatives, and the charges involved.

  • You have the right to be informed of the treatment plan for you and give informed consent to your treatment.

  • You have the right to accept or refuse any medication, investigation, or treatment, and to be informed of the likely consequences of doing so.

  • Your privacy is considered and respected by our team.

  • You and your authorised/legitimate carers or representatives have the right to be informed about the procedures for making complaints, and the process of managing and responding to your complaints by our team.

  • You have the right to access your own dental records.

  • You have the right to obtain a copy of your dental records in accordance with the law

  • You have the right to obtain a second opinion.

 

We also advise you to observe your responsibilities:

  • Ask questions and seek clarification in order to understand your dental condition and/or treatment.

  • Give our team as much information as you can about your present health, past illnesses, any allergies and any other relevant details to enable them to provide the most appropriate and effective care.

  • Follow the prescribed and agreed treatment plan, and conscientiously comply with the instructions given.

  • Should not ask our team to provide incorrect information, receipts or certificates.

  • Keep any appointments that you make or notify our team as early as possible if you are unable to do so.

  • Meet your financial obligations in a timely manner.
